    Unexplained infertility is a condition where a couple is unable to conceive despite having regular sexual intercourse without contraception for at least a year. This diagnosis applies to both men and women, with all reproductive structures, ovulation, and semen analyses appearing normal.

    What Is Progesterone Resistance?

    Progesterone resistance is when the body’s tissues show decreased responsiveness to the hormone progesterone. This hormone plays a role in female reproductive health, including the regulation of the menstrual cycle and preparation of the uterus for pregnancy. When the body exhibits progesterone resistance, it can lead to complications in different gynecological conditions, one of which is endometriosis.

    Endometriosis is a disorder where tissue similar to the lining inside the uterus grows outside it. Progesterone resistance is often observed in women with endometriosis. It is thought to contribute to the clinical challenges associated with this condition, including pain, infertility, and inflammatory responses.

    What Are Ways to Treat Progesterone Resistance?

    Understanding the various ways of treating progesterone resistance can provide solutions for those dealing with this challenging condition. Here are multiple ways of treating progesterone resistance:

    Immunohistochemistry Test

    An immunohistochemistry test can identify endometriosis, progesterone resistance, and endometritis. It uses markers such as BCL6 and beta-three integrins to detect these conditions with high sensitivity and specificity. The test is non-invasive and offers enhanced screening for all stages of endometriosis and other conditions that interfere with implantation.

    Surgical Laparoscopy

    Surgical laparoscopy may involve making small incisions in the abdomen to insert a camera and special surgical tools. Unexplained infertility surgeons can use this method to visually identify and remove endometrial tissues that have grown outside the uterus. By eliminating these tissues, surgical laparoscopy can alleviate pain and other symptoms caused by endometriosis. It also addresses progesterone resistance by removing the tissue that does not respond correctly to the hormone. As a result, this procedure can improve fertility outcomes for those struggling with progesterone resistance.

    Hormone Suppression Therapy

    Hormone suppression therapy may involve using medication to reduce or block the production of certain hormones in the body. Hormone suppression therapy can effectively address imbalances in the body’s hormonal regulation. This is specifically estrogen dominance, which often co-occurs with progesterone resistance. In conditions like endometriosis, hormone suppression therapy can provide relief from symptoms and improve fertility outcomes. The therapy functions by blocking ovulation, thereby curbing the production of hormones that might contribute to progesterone resistance.

    What Are Other Causes of Unexplained Infertility?

    While progesterone resistance is one possible factor, there are many other causes. Here are the different causes of unexplained infertility:

    Ovarian and Testicular Dysfunctions

    In women, ovarian dysfunction can disrupt the release or quality of eggs, making it difficult to conceive naturally. These disorders may be due to various factors, including premature ovarian insufficiency and low ovarian reserve. Testicular dysfunction in men primarily affects the production and quality of sperm. Conditions such as primary testicular failure can lead to male infertility.

    Sperm and Oocyte Quality

    In females, poor oocyte quality may hinder successful fertilization or embryo development. This could be due to age-related decline, genetic abnormalities, or other unknown factors affecting the health of the eggs. For males, the fertility potential isn’t solely about the quantity of sperm but their quality. Factors such as sperm motility, morphology, and genetic integrity affect the sperm’s ability to reach and fertilize the egg.

    Blocked Fallopian Tubes

    The fallopian tubes serve as the pathway for the egg to travel from the ovaries to the uterus. Any obstruction in these tubes can prevent the sperm and egg from meeting. Several factors, including infection or benign tumors such as fibroids, can cause this. The first symptom of blocked fallopian tubes is often infertility. In some cases, tubal obstruction may also cause abdominal pain.
