एक बार की बात है, देवता और राक्षस समुद्र से अमृत निकालने के लिए समुद्र मंथन कर रहे थे, लेकिन अमृत के साथ-साथ समुद्र से विष यानी जहर भी निकला। यह जहर इतना खतरनाक था कि उसकी एक बूंद पूरी दुनिया को खत्म कर सकती थी। अब देवता और राक्षस दोनों ही डर गए कि अब क्या किया जाए। फिर वो भगवान शिव के पास गए। उन्होंने भगवान शिव को सारी बात बताई।

भगवान शिव ने फैसला किया कि वो उस विष को पी जाएंगे। भगवान शिव ने विष का घड़ा उठाया और सारा विष पी लिया, लेकिन भगवान शिव ने उस जहर को अपने कंठ में रख लिया, उसे गले से नीचे नहीं उतारा। जहर के कारण भगवान शिव का गला नीला पड़ गया और यही कारण है कि उनका नाम नीलकंठ पड़ा।
